For several years Hospice of Wake County has offered more than hospice care and has served patients well beyond Wake County. Though we are changing our name to Transitions LifeCare, we will continue to offer services such as hospice, palliative care, bereavement, etc.
We are changing what we are called, not who we are. Our mission and vision are unchanged and we remain a local, non-profit serving patients regardless of their ability to pay.
Our core brand name, Hospice of Wake County, no longer accurately reflects all that we do. We're not just hospice. And we serve more than Wake County. Our service territory extends well beyond Wake County so that we now help more patients and families in Durham, Johnston, Franklin, and Harnett counties, and in the communities of Chapel Hill and Carrboro.
Rest assured that even though our name is changing, our expertise, compassion and commitment to helping families facing a serious or terminal illness remain the same. Our mission and vision are unchanged. We are still a non-profit organization with a local board of directors. We have not been bought by a large, for-profit corporation.
Going forward, Transitions LifeCare is our single brand name, or umbrella name and our service lines have corresponding names: Transitions HospiceCare, Transitions PalliativeCare, Transitions HomeHealth, and Transitions GriefCare. NOTE: our core service line of hospice is NOT going away.
